Advance Care Planning
Advance Care Planning means making a plan to make sure your wishes are followed by your family and by medical and health professionals, even if you can no longer speak for yourself. The plan includes an Advance Care Directive, which is a legally binding document to say what kinds of medical treatment you do or do not agree to. The rest of the plan is your wishes for how you would like to be treated. It is different from your Will as it does not cover any of the things in your Will.
We can provide a printed copy of the Advance Care Plan and help you to understand who to talk with about your medical care and what sort of things you might want to include in your Care Plan.
